Goshen DDC is seeking a REGISTERED NURSE – RN experienced in Adult/Child Habilitation Waiver in our Omaha office. Our busy program works closely with the State of Nebraska Medicaid Waiver (DD) to provide quality services to individuals with developmental disabilities and foster care. SKILLS/QUALIFICATIONS

Education

  • Bachelor’s Degree and/or a minimum of 5 years experience in a related field.

Competencies

  • Must be able to operate personal computers and office equipment. Knowledge of Microsoft Outlook, Word, and Excel programs, as well as an understanding of the importance of confidentiality pertaining to member, employee, and company information, is required.
  • Key traits include attention to detail, excellent written and verbal communication skills, the ability to work cooperatively with a diverse group of people served, including their stakeholders and community members, as well as the ability to respond to stressful situations in a calm and professional manner.
  • Must reflect a positive image of the agency while in the community and be willing and able to work a flexible schedule

Requirements:

  • Valid Driver’s License
  • Current vehicle Insurance upon request
  • Pass a State & National background screen
  • Current license in good standing with the State of Nebraska as a registered nurse
  • DD Wavier experience
  • Demonstrate strong interpersonal and written communication skills

  • Be able to pass a physical (including lifting 70 pounds), drug screen, and criminal background check (APS/CPS Registry check included)

ESSENTIAL FUNCTIONS

  • Conduct routine assessments and seek medical attention for members when needed
  • Assist with the coordination of member medical appointments and care, including attending appointments, if a member needs warrants direct involvement
  • Act as a liaison between external medical professionals, the member served, their team members, Discovery Living personnel, and parents/guardians
  • Routinely make on-site visits to member homes
  • Take physician orders and communicate information to needed parties
  • Accurately and promptly complete and review required documentation
  • Provide agency staff with accurate and timely information regarding medical and health-related issues
  • Provide medical training, and act as a resource, to agency personnel and team members
  • Attend member team meetings as requested
  • Participate in medication administration and training of Medication Managers
  • Respond and assist with critical member medical issues, even when those issues arise outside of routine work hours
  • Provide direct member care and assistance as needed
  • Perform other duties as requested
  • Communicates health/nursing needs and/or recommendations to staff, other health providers, and families.
  • Coordinates hospital admissions, discharges, and follow-ups as needed.
  • Checks program/service areas on a frequent but not less than a quarterly basis to ensure the necessary medical/nursing supplies are available and in ample supply.
  • Assists on medical/health appointments as needed.
  • Provides direct care services as needed within the treatment or habilitation environment
  • Ensures the provisions of nursing/health services as outlined in the state and federal regulations and divisional directives.
  • Ensures the provision of nursing/health services as ordered by physicians and other health professionals/consultants.
  • Ensures the provision of nursing services/programs as outlined in the Service Plan.
  • Ensures the provision of needed adaptive equipment.
  • Monitors individual health status through the provision of general nursing services as outlined/ordered.
  • Monitors incidents involving individuals, ensure the incident report is completed timely and accurately, and examines the individual.
  • Ensures aggressive management of individual health needs when indicated by adverse changes in the medical/health status of the individual.
  • Works closely with home/facility staff in consulting on psychotropic medications, side effects, and planning agenda for medication reviews.
  • Monitors height and weight of persons served.
  • Administers and/or oversees medication/physical treatments as ordered by the physician and provides an assessment of adverse effects/results.
  • Reports pertinent information and changes in individual health status to attending physician/ health professionals and to the Team as needed in a timely manner.
  • Screens ill individuals and refers them to a physician if needed.
  • Orders, reviews, monitor, and discontinue medication as ordered by a physician. Dispose of unused medication per policy, reporting adverse reactions.
  • Assures that monthly Medication Administration Records (MAR’s) are in each individual’s record.
  • Reports possible communicable diseases or infections and works closely with nursing staff and/or other health care professionals, staff, and family in assessing and treating the situation.
  • Ensures the routine documentation of individual health status in nurse’s notes.
  • Transcribes or ensures the transcription of physician phone orders and other medically related reports, prior to filing in the individual’s chart.
  • To maintain the confidentiality of all resident care information.
  • Other duties as assigned
