Responsible for the management and motivation of staff. Ensure individuals in residential services receive quality care, treatment, and community integration. Work with an ISP team which includes State Coordinators, family members, and guardians. Responsible for the implementation of service reviews; assessments; programs and monitoring; billing; staff scheduling and time monitoring. Works closely with the Quality Assurance team to identify issues and develop strategies for improvement. Responsible for reporting all pertinent information/concerns to the Quality Assurance and Service Directors.

Areas of Responsibility:

Manage operations in areas of responsibility.

  • Ensure and monitor quality assurance and documentation in the residences.
  • Ensure the medical and behavioral health needs of our individuals are met, including medication administration and documentation.
  • Support the Shared Living Providers and the individuals in their care.
  • Counsel, develop, evaluate, and discipline (as appropriate) staff.
  • Implement and enforce GOSHEN DDC practices and procedures.
  • Work cooperatively with the Administrative Team for the welfare and development of individuals receiving care.
  • Keep Quality Assurance and Service Directors informed of significant and relevant information.
  • Communicate weekly with guardians and or family members of individuals served.
  • Communicate with schools, the state, and other members of the individual’s team on an ongoing basis.
  • Mentor the SLP and DSP, providing assistance as needed.
  • Maintain residential schedule and oversee daily, weekly, and monthly integrity standards.
  • Oversee the individual finances in homes.
  • Ensure month-end documentation and hours are complete and accurate and submitted in a timely manner.
  • Participate in health and nutritional monitoring and development of programs and supports.
  • Lead, as well as provide support and supervision to a team of assigned staff in support of persons served and ensure the implementation of their goal.
  • Establish high standards of conduct and job performance for the team, and lead by example.

  • Effectively assign, direct, control, evaluate performance, coach and hold the accountable assigned team.
  • Conduct effective staff and one-on-one employee meetings.
  • Develop and/or support the scheduling for direct care staff to ensure supports are available to persons served.
  • Communicate staff schedule needs/changes in a timely manner and arrange for replacements.
  • Ensure tracking and billing of direct service hours and ensure accuracy efficiency and cost-effectiveness.
  • Assist in the hiring process and coordinate and conduct onboarding of new staff, as well as be involved in ongoing training opportunities in the form of regular staff and small group team meetings.
  • Maintain open communication channels and keep leadership aware of client and/or staff support needs.
  • Coordinate with direct care staff and ensure the scheduling of clients’ medical appointments, including transportation.
  • Supervise the area of medication management.
  • Practice compliance with employment law guidelines and mandates.
  • Oversee the execution of medication management plans.
  • Support staff in the implementation of behavioral plans.
  • Ensure timely documentation of client interactions and support interventions in the form of progress notes, incident reports, communication logs, and others as needed and instructed by direct support staff.
  • Be responsive to and provide emergency intervention for all persons receiving support.
  • Ensure clients receive assistance with social security including work incentives, wage reporting, benefits counseling, and assist with the management of personal finances (as needed and desired).
  • Ensure support for persons served in locating and securing adequate and desired housing options, including advocating on their behalf with landlords, assisting in arranging to move in/out of apartments, and supporting consumers’ desires to purchase their own homes.

Ensure clients receive optimal service.

  • Ensure individual needs are assessed; habilitation plans are implemented as designed.
  • Ensure staff is using appropriate intervention strategies and positive reinforcement with individuals.
  • Participate in IPP and Intake meetings.

Knowledge, Skills, and Abilities

  • Knowledge of: Current practices in the field of community-based services for persons with intellectual disabilities and other services for persons with developmental disabilities and foster care
